Participant Pick-Up Authorization

You or an authorized adult must sign your child in and out of this program. Please provide the names of authorized adults who have your permission to pick up your child. Changes to this list will require written documentation.

Photography Release

Photo Release
Workshops are documented so students, teachers, and the general public may view the outcomes of arts education. In-class photography or video may be used educational purposes at conferences and seminars, education program brochures printed for The Columbus Museum, and assessment evidence for foundations or grantors that fund the project. Students are never identified using their full names. Your child is welcome in this art program even if you do not allow them to be photographed.

Agreement & Release of Liability

I am the parent or guardian of the minor child listed on this agreement, referred to in this agreement as “my child”, who is going to participate in the Kids Workshop sponsored by The Columbus Museum.

I understand that The Columbus Museum has a Board of Trustees, employees, and volunteers who carry out their programs. This agreement applies to anyone acting on behalf of The Columbus Museum. The Columbus Museum will take all reasonable steps to provide a safe environment for all persons who participate in their activities. On behalf of my child and myself, I hereby waive, release, and discharge any and all claims and damages for personal injury, death, or property damage, other than those specifically excepted below, which my child may sustain or which may occur as a result of my child’s or my participation in these activities. I understand that:

  1. This release is intended to discharge in advance The Columbus Museum their Boards, employees, and volunteers from any and all liability, except for their sole negligent or intentional acts, connected in any way with the participation of my child in these activities.
  2. The activity in which my child participates may be of a strenuous and/or physical nature.
  3. I hereby assume any and all risks of injury, death, or property damage and release and hold harmless The Columbus Museum and their Boards, employees, and volunteers, except for their or their agent’s sole negligent or intentional acts.
  4. This agreement and assumption of risk is to be binding on the heirs and assigns of my child and myself.
  5. I will hold The Columbus Museum harmless from any loss, liability, damage, cost, or expense, including cost of litigation, which may be incurred as a result of any injury and/or property damage which my child may sustain or cause while participating in said activities, unless resulting from their or their agent’s negligent or intentional acts.
  6. I will make good any loss or damage The Columbus Museum may have to pay if any litigation arises on account of any claim made by my child or anyone on my child’s behalf, unless resulting from their or their agent’s negligent or intentional acts.
  7. In the event that my child requires medical or surgical treatment while under the supervision of The Columbus Museum, personnel of those organizations in connection with the described activity may authorize necessary emergency medical treatment.

I certify that I am the parent of the above named child or that I have custody or am the legal guardian of the above named child by court order and that my child is physically able to participate in the described activities. I have carefully read this Release Agreement and fully understand its contents. I am aware that this is a release of liability contract between The Columbus Museum and myself, and I sign it of my own free will.

In an effort to ensure each of our students have a safe and enjoyable experience we ask that you review our expectations with your child. Please sign and date the bottom of this form to indicate that you have reviewed and understand the information given. Questions should be directed to the Youth and Family Programs Manager at 706-748-2562 ext. 6520. A copy of the signed form will be available upon request.

Children attending the Kids Workshop at The Columbus Museum are expected to:

  • Be responsible for their actions
  • Respect each other and the environment
  • Base all interactions on honesty
  • Care for themselves and others around them
  • Make appropriate choices for themselves

Behavior which is considered to be unacceptable includes the following:

  • Endangering the health and/or safety of a child, staff member, volunteer or animal
  • Stealing or damaging property
  • Leaving the program area without permission
  • Continuing to disrupt a program in any way
  • The use of profanity, vulgar, or obscene words and gestures
  • Possession of a weapon
  • Bullying and mocking
  • Any action which may make another student uncomfortable

The following are steps that will be taken to ensure all students have an enjoyable experience:

  1. If an unacceptable choice is made an initial verbal reminder will be given.
  2. If the behavior is not resolved these steps will be taken, staff will redirect the student to a more appropriate behavior
  3. Staff will have a private discussion with the student about the behavior if it continues.
  4. Staff will document the situation and include the problem, possible cause of the problem, and any corrective measures taken. Education Department staff will be notified.
  5. If behavior persists, parent/guardian will be notified.
  6. Staff and Education Department member will ask for input from the parent/guardian in regards to the behavior.
  7. Staff and Education Department member will check back with parent/guardian at the end of the following day to report on behavior.
  8. If problem persists, becomes excessive, and the child continues to disrupt the program, the Education Department reserves the right to expel the child from the program. No refund will be given.
  9.  If a child’s behavior threatens the immediate safety of the individual, other children, or staff, the parent/guardian will be contacted and expected to pick the child up immediately.
